This form was used for notifying damage to buildings covered by the act. This particular example was completed on behalf of Miss Isabel Haswell of "Richborough", South Eastern Road, Ramsgate, Kent and relates to repairs to the property following damage received in an air raid on the 14th June 1941.

That night, enemy mine-laying aircraft were operating over the Thames Estuary. It is possible that a strong wind from the north-east resulted in three aerial parachute mines drifting over Ramsgate. Fifteen houses were demolished in Wilfred Road and four people killed. Considerable damage was caused by the blast over a wide area, with a further forty houses seriously damaged and over two hundred suffering minor damage.
